I recognize that the risks of accident and/or injury are possible consequences of participation in any activity, and that no amount of reasonable supervision will prevent every and all type of injury. I have carefully considered how the possible consequences of injury may impact the lives of my family members, and choose to accept the risk. In accepting this risk, I expressly and explicitly, release the Rose Valley Swimming Pool Corporation, their employees, officials or agents of any and all of the foregoing, pertaining or related to, arising from, in any manner, injuries as a result of our use of the Rose Valley Swimming Pool Corporation facilities.

As is the case with any facility outside of your home, use of facilities at the Rose Valley Swim & Tennis Club may expose you to COVID-19 and other associated viruses. Your decision to access Club facilities is made knowingly and voluntarily by you with full knowledge of this risk. If you do decide to use the Club, your use of these facilities shall mean that you are accepting the risks associated with the conditions outlined above and acknowledge that the Club is not responsible should you contract the virus.

In the event anyone in my household is in the need of emergency first aid treatment, I hereby authorize agents, designees or employees of the Rose Valley Swimming Pool Corporation to render any first aid treatment. In the event that anyone in my household requires additional medical attention, I grant permission to have anyone in my household transported to an emergency medical facility to receive medical treatment. In the event that I or my designated emergency contact person cannot be immediately contacted, I give my consent to allow any necessary emergency medical and/or surgical treatment to be performed on anyone in my household.
